We offer infant, child, adolescent, and adult recommended immunizations. Immunizations help protect against diseases and are one of the most important ways that people can protect themselves and their loved ones against serious diseases. Children birth through 18 years of age may be eligible for free childhood immunizations through the Vaccines For Children Program. This program offers recommended immunizations for children who are on AHCCCS or KIDSCARE, have no other insurance, are Native American, or have no immunization benefits on their health insurance plan.
Confidential Teen Services
Canyonlands Healthcare provides confidential services to teens and young adults. Our providers are committed to serving the adolescent community and tailors our services to meet their unique health care needs.
This program provides assistance with prescription coverage for patients who have no prescription coverage. There are eligibility requirements. Please bring your medications to every visit.
Chronic Care for Diabetes, Asthma & Hypertension
Information is available about nutrition guidelines, exercise, self care at home, monitoring medications, and disease prevention. The goal is to improve patient control of their health condition.
Black Lung Clinic Program
Black Lung Clinic Program will establish health education, outreach and treatment strategies for coal miners residing in the northern Arizona remote communities of Kayenta and Page. These efforts target miners and their families who present with primary and secondary symptoms as well as medical conditions associated with black lung disease.
Well Woman HealthCheck
The Well Woman HealthCheck program is for women over 45 years of age who are uninsured or under insured. Qualifications are based on family income. The WWHC consists of a yearly visit, pap smears, clinical breast exams, and a mammogram. Additional tests may be covered if necessary.
