Canyonlands offers special programs designed to improve access to care for those who face financial barriers and need assistance. Canyonlands Healthcare has been designated as a Federally Qualified Health Center (FQHC) and as such is supported by federal grants under the U.S. Public Health Service Act.
Sliding Fee Programs
Canyonlands Healthcare offers a sliding fee program designed to make quality health care available to people who lack adequate insurance and have limited income. Qualifying members of the program have access to all the quality health care services at Canyonlands Healthcare locations at a reduced rate. Based on individual financial circumstances, the program allows members to pay variable rates from a minimum of $5.00, up to 75% of the office visit charges.LEARN ABOUT THE SLIDING FEE PROGRAMS

KidsCare – (CHIP)
AHCCCS offers health insurance through KidsCare – Arizona’s Children’s Health Insurance Program (CHIP) for eligible children (under age 19) who are not eligible for other AHCCCS health insurance. For those who qualify, there are monthly premiums.
Vaccines for Children Program
This program offers recommended immunizations for children 0-18 years old who are on AHCCCS or KIDSCARE, have no other insurance, are Native American, or have no immunization benefits on their health insurance plan. Those with private insurance that fully covers immunization services are not eligible for the Vaccines For Children Program. Please bring your Immunization Record Booklet to every clinic visit to ensure the record is complete and up-to-date.
Arizona Health Care Cost Containment System (AHCCCS) is Arizona’s Medicaid agency that offers health care programs to serve Arizona residents. Individuals must meet certain income and other requirements to obtain services.
Our financial counselors can help people with limited income to enroll in AHCCCS over the internet. This provides faster processing and shorter wait times. There is no separate interview with an AHCCCS or DES caseworker needed.
This program provides assistance with prescription coverage for patients who have no prescription coverage. There are eligibility requirements. Please bring your medications to every visit.

Medicare Part D
Speak with a Canyonlands Financial Representative to help you with your Medicare Advantage Plan (Part C) or Medicare Prescription Drug Coverage (Part D).
Well Woman HealthCheck
This program helps low-income, uninsured and underinsured women gain access to age appropriate breast and cervical cancer screening and diagnostic services including clinical breast exams, mammograms, Pap & HPV tests and pelvic exams. Diagnostic testing such as ultrasounds, biopsies and other tests are also available if screening results are abnormal. Referrals are made for treatment as necessary.
