Affordable
Canyonlands provides integrative care at Arizona and Nevada locations that are Federally Qualified Community Health Centers. Each center ensures patient care regardless of the ability to pay.
Our Financial Representatives assist patient the application process for insurance coverage under the Affordable Care Act as well as eligibility for Medicaid, CHIP, Medicare, Vaccines for Children and other programs.
In addition, we offer special programs designed to improve access to care for those who face financial barriers and need assistance. Our sliding fee program is designed to make quality health care available to people who lack adequate insurance and have limited income. Qualifying members of the program have access to all the quality health care services at Canyonlands Healthcare locations at a reduced rate.
Accessible
Our community health centers offer comprehensive healthcare to patients in Page, Beaver Dam, Chilchinbeto, Clifton, Duncan, Globe, Mesquite (NV) and Safford.
Remote Blood Pressure Monitoring
Our Remote Blood Pressure Monitoring Program is one of our services that facilitates accessibility. The program is free for Canyonlands patients with uncontrolled hypertension. Another free service, the Page and Chilchinbeto Diabetes Education Program utilizes community resources to enhance patient engagement in their diabetes management.
Telehealth for Chronic Conditions
For patients suffering from chronic conditions like thyroid, diabetes, hypertension, or heart disease, telehealth provides active communication with their primary care provider and fewer trips to the clinic.
Family Planning
Family planning services can also be assisted with telehealth. After reviewing lab tests, prescriptions and other components of care, the provider can speak with the patient, make assessments and determine when the next in-person visit will be required.
Mental Health Services
Telehealth has become a lifeline for patients dealing with anxiety and stress. Face-to-face support from a behavioral health specialist is confidential and can be provided in the privacy of the home or office.
