We are a local community healthcare center for the greater Clifton, Arizona area providing Primary Care, Family Planning, Pediatrics, Behavioral Health and Medical Lab services.   We offer affordable health care on a sliding fee scale basis and financial assistance with state and federal programs. Services are provided to all persons without regard to age, sex, race, religion, ethnic origin, or any other factor.

Appointments

Patients are encouraged to make an appointment. However, unscheduled (walk in) patients will be seen in the order they arrive and as they can be worked into the schedule, unless the urgency of the medical condition dictates otherwise. The nature of the integrated model allows for patients to be seen by a provider without having a scheduled appointment. It may not be feasible to address all medical concerns in one visit and appropriate follow up visits may be required.

Hospital Admission Policy

If a patient is in need of hospital admission, they will be referred to the Emergency Room at Mount Graham Regional Medical Center to be treated and admitted. CHC staff will follow up with the hospital to ensure the patient is followed and appropriate follow up services are received in the continuum of care.

Holidays
CHC sites are closed 7.5 days per year for holidays which include: New Year’s Day (January 1); Memorial Day (last Monday in May); Independence Day (July 4); Labor Day (first Monday in September); Thanksgiving (fourth Thursday and Friday in November); Christmas Eve afternoon (December 24); and Christmas Day (December 25). If one of these holidays falls on a day the Clifton Clinic is open, the center will be closed in observance of the holiday.

Note: Clinics may or may not be closed the week of Christmas or New Year’s depending on staff availability.

After Hours
If a patient needs to seek care after established clinic hours, they are informed to go to the closest facility that offers emergency care 24 hours a day. In the Clifton community this is Mount Graham Regional Medical Center and Gila Health Resources. Patients have the ability to access providers after hours through a contracted call service.
