Our CHWs are community-based team members who help you navigate care, stay supported between visits, and connect to local resources so you don’t have to figure it out alone.

What is a Community Health Worker (CHW)?

A Community Health Worker (CHW) is a trained member of our care team who helps you and your family navigate healthcare in real life. CHWs are often from the communities they serve, so they understand local barriers like transportation, schedules, cost, or finding the right resources.

CHWs don’t replace your doctor—they help you connect the dots between your visits, your goals, and the support you need to stay healthy.
